Registration Form

You can either download the Summit 2010 registration form and post it in, or complete the online form below. The download form contains information about the speakers, the schedule and accommodation.

Instructions

1. This registration form uses the PayPal web site to collect the delegate information and process the payment.
2. You will be able to pay using Visa, Mastercard or a PayPal account.
3. Please complete the registration details for each Summit Delegate that you are paying for.
4. You will add each delegate, one at a time, to your "shopping cart", by clicking the appropriate "Registration ->" button.
5. This will take you to the Paypal web page, displaying your "shopping Cart". Each delegate will be represented by an item in your shopping cart.
6. If you make a mistake, you will need to remove the delegate from the "shopping cart" and enter the information again.
7. On the Paypal Shopping Cart web page, click "Continue Shopping" until you have added all of the delegates. This will return you to this registration page.
8. When you have added all delegates, check that all details are correct and click "Proceed to Checkout" on the Paypal web page, and pay using your chosen method.
9. You will then be brought back to the Youth Alive web site, and the Youth Alive office will be notified of your registration.
